Nothing in these Terms and Conditions shall create or imply any agency, partnership, joint venture, employee-employer relationship, or franchisor-franchisee relationship between you and us. If any provision of these Terms and Conditions is deemed invalid or unenforceable, that provision will be removed, and the remaining provisions will remain in full force and effect. You may not assign this agreement, but we may assign it at our sole discretion. Our failure to act regarding a breach by you or others shall not be considered a waiver of our right to act regarding subsequent or similar breaches. The Contracts (Rights of Third Parties) Act 1999 does not grant any third party the right to enforce any term of this agreement. This does not affect any right or remedy generally available to such a third party in law or otherwise.

Please note that Stripe doesn't convert the transaction prices, so if you're in the USA, for example, it won't display the equivalent amount in USD. We are not responsible for any differences in charges for international currencies. When you make a purchase on www.lifestylemedicalclinic.co.uk, the amount will be charged to you in your host country's currency at the equivalent rate based on the GBP price. All currency conversions and calculations during the payment process are handled by Stripe.

Errors, Inaccuracies and Omissions

This applies whether the claim is based on contract, negligence, strict liability, or any other legal theory, even if we've been advised of the possibility of such damages. Some states or jurisdictions may not allow the exclusion or limitation of liability for consequential or incidental damages, so our liability in those cases will be limited to the maximum extent permitted by law.

 

All the General Practitioners and healthcare professionals at Lifestyle Medical Clinic have the necessary professional medical insurance for their work.

 

There are also other medical specialists, practitioners, and therapists who provide services at Lifestyle Medical Clinic. They may specialise in areas like dermatology or ultrasound. While we assist in booking appointments, answering inquiries, and handling invoices on their behalf, they are responsible for their professional services. If you have any complaints about their care, please direct them to the healthcare professional who provided the service.

 

Please take care of your personal belongings when you visit Lifestyle Medical Clinic. We are not liable for compensating you for any loss or damage (other than death or personal injury) resulting from our negligence.

Indemnification

You agree to indemnify, defend and hold harmless Lifestyle Medical Clinic and our parent, subsidiaries, affiliates, partners, officers, directors, agents, contractors, licensors, service providers, subcontractors, suppliers, interns and employees, harmless from any claim or demand, including reasonable attorneys’ fees, made by any third-party due to or arising out of your breach of these Terms of Service or the documents they incorporate by reference, or your violation of any law or the rights of a third-party.

Severability

In the event that any provision of these Terms of Service is determined to be unlawful, void or unenforceable, such provision shall nonetheless be enforceable to the fullest extent permitted by applicable law, and the unenforceable portion shall be deemed to be severed from these Terms of Service, such determination shall not affect the validity and enforceability of any other remaining provisions.

GP and Lifestyle Medicine Services

Consent

When you make an order or book an appointment with Lifestyle Medical Clinic Ltd, you confirm that you are at least 18 years old (or have your parents' permission) and agree to these terms and conditions. These terms and conditions will apply to all orders or appointments you make at Lifestyle Medical Clinic Ltd for buying or receiving any products or services.

Digital products

​

Our digital downloads provide you with immediate access to copyrighted and proprietary material. Once you download or open them, they are considered "used." Unfortunately, this means that we have a strict policy of no refunds or exchanges if you are dissatisfied with the product.

 

We cannot be held responsible for refunds or exchanges due to technical issues on your computer or iPhone, such as printer malfunctions, difficulties installing Adobe Acrobat Reader (or alternative PDF reader), or problems related to internet connectivity.

Services

​

Lifestyle Medical Clinic is committed to providing private GP and Lifestyle Medicine services to our clients during our clinic's opening hours. Our consultations for GP services are conducted by fully qualified General Practitioners registered with the General Medical Council. For Lifestyle Medicine services, consultations are conducted by fully qualified Lifestyle Medicine Physicians or Practitioners.

 

Please be aware that our clinic does not offer treatment for urgent medical issues that require immediate attention, such as breathing problems, severe bleeding, collapse, or intense pain. In such cases, we strongly recommend visiting your local urgent care centre.

 

For medical emergencies, always dial 999. If you need medical attention outside of our operating hours, please call 111 or visit local services such as A&E (Accident & Emergency) or an urgent care centre.

 

Appointments are available within our clinic's opening hours and can be booked online, by phone, email, or in person. Usually, patients can see their preferred doctor (subject to availability), but in emergencies or for urgent appointments, this may not always be possible. Appointments can be scheduled up to 4 weeks in advance or on the same day. If a same-day appointment is not available, rest assured that a doctor will discuss your medical concerns over the phone and arrange appropriate medical attention as necessary. Patients residing outside our local area may be directed to seek care from a local service based on their medical needs. Please note that patients must be within the United Kingdom to receive medical care, including email, telephone, or video consultations.

 

In certain circumstances beyond our control, the availability of services at the clinic may be limited. Our clinics are located at 392 Richmond Road, London, TW1 2DY. In the event of emergencies such as fire or flood, services may be temporarily provided at an alternate location. If the clinic permanently moves to a new location, we will provide at least 3 months' notice to all our clients.

Pre-Consultation Questionnaires

​

When booking Lifestyle Medicine and private GP services, it may be necessary to complete a questionnaire before your appointment. For most Lifestyle Medicine services, an extensive questionnaire will be required, which may take about 1 hour to complete. It's important to submit these questionnaires at least 1 week before your appointment. To ensure enough time for completion, we recommend booking appointments in advance.

 

These questionnaires play a crucial role in providing a thorough and comprehensive consultation/service. If the questionnaires are not completed beforehand, your healthcare professional may need to spend valuable consultation time completing them, which could reduce the time available for a more personalised service.

 

For private GP services, incomplete questionnaires may limit our ability to treat you effectively and prioritise your needs. This could result in the cancellation of appointments if we are unable to provide the necessary care, which may result in the loss of a deposit or administrative fee.

Clinic opening hours

​

The clinic’s core opening hours are 09:00 to 17:00 Monday to Friday. The clinic is usually closed on Saturdays, Sundays and public holidays and Easter Saturday. Our in-person appointment hours are Friday 08:00 to 08:00.

Home Visits (within clinic opening hours)

​

Lifestyle Medical Clinic may offer home visits for Lifestyle Medicine services and NOT for GP services. Please contact us to discuss this with one of our doctors or practitioners. If a doctor or practitioner agrees a home visit is required it will be arranged as soon as possible depending on the availability of the doctor or practitioner to attend. We usually only undertake home visits for clients within a 10 mile radius of TW1 2DY. Please see our price list for the current fees. 

Care Outside Clinic Opening Hours

​

Clients are able to access GP or Lifestyle Medicine services ONLY in clinic opening hours. Clients of the clinic who are in need of urgent medical attention outside of clinic operating hours should firstly contact local services such as urgent care, 111 or NHS GP.

In a medical emergency, patients may be directed to call the emergency services on 999 or attend their nearest Accident and Emergency department. 

Pre-Payment Policy

​

To secure your appointment with a doctor, practitioner, nurse, or healthcare assistant, we kindly ask for pre-payment or a deposit. For new patients, the pre-payment amount is typically up to 100% of the standard appointment charge, excluding any additional costs for medication, tests, letters (such as sick notes or referrals), or procedures. Patients who have missed previous appointments will be required to make full payment in advance to secure a new appointment.

 

At the end of your consultation, the remaining consultation fee and any additional costs for tests, medications, or services should be paid in full directly to the doctor, nurse, or healthcare assistant.

Additional Charges

​

We want to keep you informed about any additional charges before they are applied. You can find prices for specific items on our website or by reaching out to us directly. If you have any questions or concerns about the fees, please feel free to ask for clarification. It's important to note that there may be additional fees for vaccinations, laboratory services, procedures, telephone consultations outside of our regular hours, and home visits. These fees are due immediately after the service is provided, and in some cases, payment may be required in advance for certain consultations or services/products.

Late policy

​

We understand that unexpected travel issues can arise, causing delays. To ensure that we can provide the best service to all our patients, we kindly ask you to contact us if you're running late. This way, we can make arrangements to accommodate you and ensure that our doctor, nurse, or healthcare assistant can still see you.

However, if you arrive more than 5 minutes late for your appointment without prior agreement, and seeing you would cause a delay for our next patient, we will consider it a missed appointment. 

 

Our clinics are often busy, and this policy is in place to maintain our appointment schedule and avoid inconveniencing other patients. If we determine that you have missed your appointment, we regret that we won't be able to refund any pre-payment, deposit, or full payment made.

Rescheduling

​

For our services, you may reschedule your appointment free of charge up to 48 hours before the time of your appointment. You can do this via the link in your email booking confirmation.

 

If your booking is less than 48 hours before your scheduled appointment for GP services you may reschedule but you will lose any pre-payment or deposit paid for your initial appointment.

Cancellation Policy

​

If you are unable to attend your appointment you agree to let us know at the earliest opportunity. Appointments can be cancelled up to 48 hours before your scheduled appointment time and any deposit paid will be refunded minus our administration charge of £15. You can do this via the link in your email booking confirmation. If you cancel your appointment or reschedule less than 48 hours before the scheduled appointment time you will not be eligible for any refund on any pre-payment. Bookings made less than 48 hours before the scheduled appointment time which are cancelled will forfeit any pre-payment or deposit.

Promotional Pricing

​

Appointments booked during a promotional period which are rescheduled, or cancelled and subsequently rebooked, will not be eligible for any promotional rate if the promotion has since closed.

 

All appointments costs are charged at the prevailing rates at the time of last booking. Appointments rescheduled or cancelled and rebooked are charged at the rates on the date of last booking.

Failure to Attend

​

If you fail to attend your appointment or are more than 5 minutes late to your booked services appointment or cancel your appointment within 48 hours of the scheduled start date you will be deemed to have missed your appointment. You will not be eligible for a refund of any pre-payment or deposit.

 

We will require full payment in advance from any patients deemed to have abused its terms and conditions, or who has previously failed to attend, cancelled or rescheduled a booked appointment with less than 48 hours’ notice.

Refunds

​

Refunds for eligible pre-payments or deposits will only be made to the card originally used to make the original booking. We will process your refund within 7 days of the request. Once we have processed your refund please allow up to 10 working days for the refund to appear on your statement as the time taken by your bank to credit the refund on your statement is outside of our control.

 

We cannot refund any medications, vaccinations, tests or treatments once they have been administered or dispensed.

Administration Fee

​

Appointments and home tests that qualify for a refund are subject to an administration cost of £15 which will be deducted from the amount refunded. This is to cover our reasonable costs.

Unpaid Fees

​

Lifestyle Medical Clinic Ltd may terminate this agreement if any fees remains unpaid 30 days after its due date for payment. Fees for other services are payable at the time of provision. In the event of a client having an unpaid invoice over 30 days Lifestyle Medical Clinic Ltd may refuse to provide further services until the invoice has been settled. 

National Health Service (NHS) Care

​

Clients are encouraged to continue their registration with an NHS GP if they are entitled to NHS care. The clinic will liaise with any other healthcare professionals involved in your care, with your consent, as needed.

Private Medical Insurance

​

If you believe you can claim the fees for your GP or Lifestyle Medicine consultation, as well as any other charges, please check with your insurance provider beforehand. Lifestyle Medical Clinic is here to assist you by providing necessary reports, correspondence, and receipts as required. Normally, you'll need to pay your invoices directly and then seek reimbursement according to your insurance policy. If you have received pre-authorisation from your insurer, we may be able to bill them directly. If you're interested in this option, please discuss it with us before your consultation.

 

In cases where pre-authorisation has been granted and Lifestyle Medical Clinic Ltd has agreed to invoice the insurer directly, if payment is not received or is declined by a third party, or if it remains unpaid in full within 3 months of the service being provided, the patient (or their responsible parent for minors) will be responsible for making immediate payment to Lifestyle Medical Clinic Ltd. To ensure prompt payment, we require all clients to provide debit or credit card details, which will be securely stored. If an insurer or third party fails to make payment, you will be notified, and the stored card will be charged accordingly. Please note that billing insurance companies directly may result in higher charges than our self-pay fees, due to the additional costs involved in the process.

Health Assessments and Blood Tests

Consent

​

When you make an order or book an appointment with Lifestyle Medical Clinic Ltd, you confirm that you are at least 18 years old (or have your parents' permission) and agree to these terms and conditions. These terms and conditions will apply to all orders or appointments you make at Lifestyle Medical Clinic Ltd for buying or receiving any products or services.

Self-Requested Tests

​

When you come in for a blood test and health assessment that you requested on your own, without one of our Lifestyle Medical Clinic doctors overseeing it, the results won't be checked by our doctor for any issues. It's important to understand that our doctors will not provide a clinical diagnosis, comment on your overall health or whether the test you've chosen is the right one, unless you've had a consultation at our clinic.

If you're at all concerned about your health, we highly recommend having a consultation with a clinician who's licensed to practice medicine in the UK before requesting any tests. We do not offer personal advice on which health assessment is right for you. Some tests in your health assessment may have age and gender restrictions based on your chosen package.

Lifestyle Medical Clinic is not responsible or liable if patients choose to have tests without consulting a registered medical practitioner first.

​

Please understand that our service does not replace a medical consultation for medical advice, diagnosis, or treatment. If you have health issues, you MUST see a clinician.

​

Keep in mind, this testing service is not meant for urgent cases because blood samples are sent to an external lab for analysis. The turnaround times on our website are just estimates for when you can expect results. Urgent blood tests should only be ordered under the guidance of a registered medical practitioner, not on a self-request basis.

Dress Code

​

Our body scan is highly sensitive and requires form-revealing clothing. Gym trousers and vest are appropriate. Please wear a sports bra if appropriate. You can find more information on how to prepare for your scan here.

We will need to access to your arm to take bloods and other examinations such as blood pressure. If you are wearing a t-shirt or vest this would provide the appropriate access.

 

Other

Force Majeure

​

If Lifestyle Medical Clinic Ltd encounters circumstances beyond its control that delay or prevent the fulfilment of its obligations under these terms and conditions, the patient will be promptly notified in writing. These circumstances, known as "force majeure events," are unforeseeable events that even an experienced service provider could not have reasonably prevented.

 

Lifestyle Medical Clinic Ltd will not be held liable for any failure to deliver services or any delays caused by force majeure events. This includes events such as strikes, extreme weather, epidemics, pandemics, terrorist incidents, system or network breakdowns, floods, fires, explosions, earthquakes, windstorms, natural disasters, wars, civil wars, riots, or government actions that prevent service delivery.

 

While we strive to provide prompt service, Lifestyle Medical Clinic Ltd cannot be held responsible for any financial losses, travel cancellations, disruptions, or refusals to travel due to delayed or unsuccessful delivery of products, services, and/or test results.

Zero Tolerance

Lifestyle Medical Clinic Ltd operates a zero tolerance policy with regard to violence and abuse and the clinic has the right to remove violent patients from the list with immediate effect and refuse future appointments in order to safeguard practice staff, patients and other persons.

Violence in this context includes actual or threatened physical violence or verbal abuse which leads to fear for a person’s safety.

​

In this situation we will notify the patient in writing of their removal from the list and record in the patient’s medical records the fact of the removal and the circumstances leading to it.
